I had a tail spot blenny in my 40 gallon last year, which ended up dying after a couple of months.  Other fish in the tank were a percula clownfish, blue green chromis, and a lyretail anthias.
I bought a white blenny (same body type as the tail spot) for my 150 gallon community tank.  Same result, dead after about 2 months.  I feed a variety of foods, so diet shouldn't be an issue.  But when I thought back about it, I never focused on watching him eat.
I'm thinking that maybe this type of blenny is a slower eater & the other fish clean house before he decides to eat anything.  Does anyone have either of these fish in their tank?  Any input on this?
Both were very sociable swimming to the front glass to greet you & perching on various objects.  I want to get another, but don't want to condemn anything to death until I can figure out the issue.  I have had no other fish deaths, so it has something to do with this family of blenny.
I also have a sailfin blenny in the 150, and he is fat and happy, but he is a different type of blenny.

I have a bi color blenny and have had great luck with him. He lives with a blue damsel, two clowns, firefish, and sixline so pretty similar to your set up. I feed a variety of food and he eats anything and everything I put into the tank but he generally will wait for some to float down to him. My blenny has been a very easy fish to keep, I don't think it would be anything you are doing.

I think that is my problem is that he would sit around and wait for the food, meanwhile my tangs and chromis would clean house.  I have to admit I never watched to make sure he was getting something to eat until it was too late (I've never had to babysit a fish before).

I have a combtooth blenny with 2 baby clowns, a coral goby, and two green chromis.

The combtooth blenny is definately the slowest/mist timid eater, but he will eat as long as I don't move about much right outside of the tank after I feed. I make sure I watch and he gets a couple big flakes or mysis every other day.
